What to do for Army MEPS?

From my experience, I believe they refer to surgery within the last 8 years. If they want your surgery record form 9 years ago, they should be able to look it up themselves. While at MEPS, I had to put down any surgery/ major illness I've had, when I had it, and where. It doesn't ask for the doctor. All you'll need is when and where.
